The TEVAR in landing zone 0 is an important procedure for the treatment of complex thoracic aortic aneurysms with aortic arch involvement. A workflow for the technique selection is proposed and the decisive steps in the implantation are highlighted with respect to intraoperative stroke prevention. The selection of patients and treatment procedure as well as the technical performance of zone 0 TEVAR can influence the outcome of patients. The available techniques for proximalization in the landing zone 0 are presented and the advantages of the individual procedures and their limitations are demonstrated. The aim was to describe the features of TEVAR in the landing zone 0, regarding the planning and technical realization of the procedure. In this context, the Ishimaru zone 0 is used increasingly more as the proximal landing zone for the treatment of complex thoracic pathologies with arch involvement than in the past decades.
